Terms of Service
Effective date: September 3, 2026.
These terms are an agreement between you and Claire Lorentz, sole trader (ABN 49 544 038 435), Queensland, Australia ("Forme," "we," "us") for use of the Forme app. By using Forme, you agree to them.
What Forme is (and isn't)
Forme analyzes photos of your meals using artificial intelligence and provides estimates of nutrient content, a nourishment score, and general educational suggestions about food.
Forme is not medical, dietetic, or health advice. Estimates are generated from photographs and are inherently approximate — actual nutritional content of any meal will differ. Forme is not designed to diagnose, treat, cure, or prevent any condition, and is not a substitute for advice from a qualified professional. If you have a medical condition, are pregnant, have or are recovering from disordered eating, or have specific dietary requirements, consult a qualified health professional before relying on any information in the app. Do not use Forme as your sole basis for health decisions.
AI-generated estimates
Forme's analysis is generated using artificial intelligence. The app may misidentify foods, ingredients, portion sizes, preparation methods, or other details visible in a photograph. As a result, nutrient estimates, nourishment scores, and suggestions may be incomplete, inaccurate, or inappropriate for a particular person or meal. You are responsible for using your own judgment when considering information provided by Forme.
Allergies, intolerances and food safety
Do not rely on Forme to identify allergens, ingredients, cross-contamination, food safety risks, or whether a food is suitable for an allergy, intolerance, medical condition, pregnancy, dietary restriction, or other individual requirement. Forme cannot determine from a photograph whether a food is safe for you to eat. Always check ingredients and food-safety information yourself and seek professional advice where appropriate.
Eligibility
You must be at least 13 years old to use Forme.
Your subscription
Forme requires a subscription ("Forme Plus") after a free trial:

Free trial: 7 days, started when you confirm the subscription. If you do not cancel before the trial ends, your subscription begins automatically and the price shown at purchase is charged to your Apple ID.

Auto-renewal: Subscriptions renew automatically (monthly or yearly, as selected) unless cancelled at least 24 hours before the end of the current period.

Managing and cancelling: You can manage or cancel anytime in your Apple ID settings (Settings → your name → Subscriptions). Deleting the app does not cancel a subscription.

Billing and refunds are handled by Apple under Apple's terms. Refund requests are made through Apple.
Prices are shown in the app before purchase and may change; changes won't affect a period you've already paid for.
Fair use
You agree not to misuse Forme — including attempting to reverse-engineer the app or its services, using automated means to access or overload our systems, using the service for unlawful purposes, or reselling access. We may suspend access that we reasonably believe is abusive.
Your content
Photos you scan remain yours. You grant us the limited license needed to process them, including transmitting them through our server to our AI provider for analysis, solely to provide the service to you. As described in our Privacy Policy, our server is designed to pass scan data through for analysis rather than retain your meal photos or analysis results.
Your responsibility
You are responsible for decisions you make based on information provided by Forme, including decisions about what to eat and whether a food is appropriate or safe for you. Where health, allergy, medical, pregnancy, dietary, or food-safety considerations apply, use appropriate independent information and professional advice rather than relying on Forme.
Our content
The Forme app, brand, design, and content are owned by us and protected by intellectual property laws. These terms don't grant you any rights in them beyond using the app as intended.
Disclaimers and liability
Nothing in these terms excludes, restricts, or modifies any consumer guarantee, right, or remedy under the Australian Consumer Law or other law that cannot lawfully be excluded. Subject to that:

Forme is provided "as is," and we don't guarantee it will be uninterrupted, error-free, or that estimates, scores, suggestions, food identifications, ingredient identifications, or portion estimates will be accurate;

to the maximum extent permitted by law, we are not liable for indirect or consequential loss, and our total liability in connection with the app is limited to the amount you paid us in the 12 months before the claim (or, where liability cannot be so limited, to resupplying the service).
Ending things
You can stop using Forme anytime (remember to cancel your subscription through Apple). We may suspend or end access for breach of these terms.
Changes
We may update these terms from time to time; the effective date above will change and significant updates will be flagged in the app. Continuing to use Forme after changes take effect means you accept them.
Emails from us
We use your email address to contact you about the service — changes to these terms, your subscription, or problems with the app.
If you tick the optional marketing box when you sign up, we may also send nutrition tips and Forme updates. That box is optional, and you can unsubscribe at any time.
Governing law
These terms are governed by the laws of Queensland, Australia, and disputes are subject to the non-exclusive jurisdiction of its courts.
